Support - Track Your OSHA Card

OSHA Outreach Card Delivery

Upon successful completion of your OSHA 10 Hour or OSHA 30 Hour courses, you must fill out the OSHA survey within the online course. You will find a link to the survey in the "end of course" instructions. It is the first icon to the right of the course and certificate information in your student interface. Once you complete the mandatory OSHA survey, your test scores and survey are sent to our OSHA trainer to review. Your information is then sent to the US Department of Labor and they will issue your card and return it to us. We will then send it to you via US mail. The process usually takes 4 to 6 weeks, but can take up to 8 weeks.

Important: As of April 1st, 2010, OSHA has asked online providers to eliminate the ability of the student to print their own certificate of completion from the website. To recieve temporary proof of completion for your job, please email our OSHA Outreach trainer of record, Mike Millsap at mike.millsap@360training.com with your name, couse taken, course completion date and email addres and he will email temporary proof of completion to satisfy your job site.

The New York State Law even addresses this by stating: Proof of completion may include but is not limited to:

  1. Copies of bona fide course completion card
  2. Training roster, attendance record of other documentation from the certified trainer pending the issuance of the card
  3. other valid proof

If it has been eight weeks since you completed the OSHA 10 Hour or OSHA 30 Hour course and survey, you may fill out the short form below and we will track your card. You should hear from us within 48 hours about the status of your card.
